What to Bring to Your Tax Appointment
- Valid Government Issued Photo Identification
- Social Security Cards for you, your spouse, and your dependents
- Form for health insurance – Form 1095-A, 1095-B, or 1095-C is needed
- Did you have creditable health insurance for all 12 months?
- Do you have health insurance for everyone you are claiming?
- Birth dates for you, your spouse, and your dependents
- Birth Certificate for all dependents
- Government Issued Photo Identification
- Proof of residency is needed for the following tax credits:
- Child Tax Credit
- Earned Income Tax Credit
- Reasonable proof of residency is

- or the prior year filed accepted professionally prepared return.

- a letter from the school, on school letter head, indicating the specific name of the child, name of the child’s parent or guardian, child’s address, and specific dates during the year child lived with the taxpayer.

- A letter, on letterhead, from a medical provider, social service agency, or place of worship. The letter must include the name of the child, name of the child’s parent or guardian, child’s address, and dates during the year child lived with the taxpayer.
- Bank Routing Numbers and Account Numbers for Direct Deposit of Refunds
- Dd you or your dependent go to college? You will need to bring:
- Form 1098-T
- Account transaction history dated 01/01/2019 to 12/31/2019
- Student Loan Interest Form 1098-E
- Wage and Earning Statements:
- Form W-2
- W-2G
- 1099-R
- Employers Final Pay stub
- Interest and Dividend Statements
- Form 1099-INT

- Contract Labor

- Consolidated Year End Brokerage Statement

- Entire booklet showing relevant income exempt from taxation
- Day-Care Year-End Statement

- Manufacturer statements for energy efficient items purchased and installed in 2019
- Out of pocket medical expenses paid in 2019 including insurance and travel
- Charitable contribution statements and receipt
